Glycemic Variability and Survival in People with Diabetes on PD

A Swedish study divided 325 people with diabetes on PD into seven groups based on glycemic variability. Using the lowest variability group as a reference point, survival was significantly better with less variability in blood sugar.

Home HD: Hospitalizations and Technique Survival

In a Swedish registry study, patients initiating PD (n=456), home HD (n=152), and in-center HD (IHD; n=608) were matched by age, sex, comorbidity, and start date. Patients using home HD had significantly lower hospital admissions and days than those doing PD or IHD (1.7, 12 days vs. 2.8, 20 days and 2.2, 14 days respectively). Home HD patients also had significantly fewer admissions for cardiovascular diagnoses or infectious disease, as well as significantly longer technique survival.

Home HD - Significantly Better Survival and Transplant Rate

A Swedish matched cohort study compared the long-term survival of 152 home HD patients with those doing PD (n=456) or standard in-center HD (n=608). Median survival for home HD was significantly better (18.5 years) than PD (15.0) and in-center HD (11.9). Home HD patients were also significantly more likely to have received a transplant.

PD and Dialysis Patient Employment

In a Swedish study, 4,734 working-age (20-60 years old) patients who survived at least one year on in-center HD or PD were compared. Patients using PD had a 4% increased probability of employment over in-center HD—as well as a 6% lower disability pension and higher work income. Read the abstract

The Secrets to Boosting Home Therapy Choice in Sweden

What factors make a choice PD or home HD more likely? A survey of Swedish patients found that receiving high quality, comprehensive predialysis information from three or more sources made a difference. Younger patients and men living with a spouse were also more likely to choose home, while those who lived close to a clinic were less likely. Once in-center, patients believed social interaction at the clinic influenced their choice.

Swedish patients who do home HD first have excellent survival

In Sweden, 128 patients who chose home HD as their first treatment and were under age 60 had 10-year survival of 75%, 20-year survival of 47%, and 30-year survival of 23%. For patients in this age group, getting a transplant did not further improve survival.
