Less Home Dialysis and Transplant in Pediatric Patients Transitioning to Adult Clinics

A poster at the annual meeting of the Association of Med-Peds Physicians showed 2025 USRDS Annual Data. Pediatric patients transitioning to adult clinics had lower rates of home dialysis and transplant and higher hospitalization rates. The poster suggests ways to improve these outcomes.
