CMS Relocates ESRD Survey Procedures and Interpretive Guidance

These documents moved from the ESRD Facility Provider Website to Appendix H (ESRD) In The State Operations Manual. The CMS-provided advance copy of Appendix H (ESRD), and links to resources in the QSO-26-09-ESRD memo to State Survey Agency Directors. Part I is the regulation and interpretive guidance. Part II includes 15 survey tasks, decision making and exit conference. Part III has surveyor worksheets, including patient and personnel interview worksheets.
