TheHub, a new health platform to improve remote monitoring of home patients has three components: “PatientHub” is for lab results/meds and messaging care teams, “CareTeamHub,” is for daily monitoring and clinical decision support, and “ProviderHub,” is for real-time physician access to medical records from devices.
