MEI’s MATCH-D Tool Helps Build a Home-first Culture

Maria Camila Bermudez, MD, from Geisinger Medical Center, integrated the Method to Assess Treatment Choices for Home Dialysis (MATCH-D) into routine clinical practice in a study to identify and assess patients for home dialysis suitability. The staff use the tool to hold a conversation with patients and work to overcome obstacles. All patients are reviewed with the MATCH-D quarterly. Read the article.
