Crossover Study from Conventional PD Dialysate to Icodextrin
A prospective study of 50 PD patients provided 6 months of standard PD fluid followed by 6 months of icodextrin—which significantly improved 24 hour UF volume, Kt/V, serum albumin, and hemoglobin.

VHA Nationwide Survey of PD Infrastructure and Clinical Practice
Increasing use of PD for Veterans is a strategic priority for the Veteran Healthcare Administration (VHA). A nationwide stakeholder survey was conducted to improve Veterans ability to choose, access, and receive excellent quality PD. A total of 27 programs responded; differences were found in PD infrastructure and clinical practices. Larger PD programs offered more services than small ones. Aligning infrastructure, standardizing best clinical practices, and quality improvement efforts are starting to increase successful PD use.

Patient Employment Rates in PD vs. In-center HD (IHD)
A study followed 18,408 PD and IHD patients who were employed full-time at dialysis start and had employer health insurance for 3 to 30 months, while 104,952 PD and IHD patients who were unemployed and did not have employer insurance were followed as well. Patients receiving IHD were more likely than PD patients to lose their employer health plans.

Chronic Inflammation and PD Outcomes
Among 111 PD patients stratified by C-reactive protein (CRP) into “normal” (<3mg/L) and “inflammation” (>3mg/dL) groups, the inflammation group had significant differences, including lower urine volume, hemoglobin, prealbumin, potassium, magnesium, and TSAT—but higher serum ferritin and WBC counts (p < 0.05). Systemic inflammation can interfere with nutrition, anemia treatment, and is linked with reduced residual kidney function.

Redesigning PD Supplies for Life Cycle Sustainability
PD tends to rely heavily on single-use plastic. Can better design change this? Researchers redesigned ten high-use PD products such as PD sets, dialysate bags, and APD machines by modelling them in Open Life Cycle Assessment, comparing cradle-to-gate carbon footprints of the originals vs. the new. All of the redesigned projects reduced carbon footprints, eight of them by >40%. It is possible to reduce emissions without altering functionality.

Meta-analysis of Factors in PD Catheter-related Infections
Thirteen studies were included in a meta-analysis that found PD catheter-infection factors included diabetes, insecure catheter fixation, >2 years on dialysis, BMI >20 kg/m2, mechanical strain, lack of proper care, irregular caregivers, and a history of catheter pulling.

Reasons for Switching from PD to HD
A retrospective analysis of 15,974 French PD patients between 2002 and 2018 found 6,835 deaths, 5,108 transfers to HD, and 3,094 transplants. Nurse-assisted PD reduced the risk of transfer to HD due to infection in the first 18 months as well as adequacy after 6 months. Surprisingly, CAPD had a higher risk of transfers to HD for mechanical issues than APD in the first 6 months—but a lower risk of issues after 18 months. Suboptimal starts had more psychosocial issues.

Early Icodextrin (ICO) Use Reduced PD-associated Peritonitis Risk
A group of 82 patients who did PD between 2011 and 2020 were divided into a group (n=21) who received ICO at PD initiation and a group (n=61) who received it later or not at all. Patients were followed until they stopped PD, died, or 3 years passed. PD-associated peritonitis and tunnel infection-free survival rates were significantly better with early ICO use.

Fat Mass May Protect Bones and Muscles on PD
Among 359 PD patients, 25% had osteoporosis, 32% had sarcopenia, 15% had osteosarcopenia, with considerable overlap between these conditions. Using body composition monitoring, fat tissue index (FTI) and lean tissue index (LTI) were measured. Low FTI—but not normal or high FTI—was linked with all three conditions, even after adjusting for age and BMI.
